The Year of Care Programme is a high profile and successful partnership between the Department of Health, Diabetes UK, NHS Diabetes and The Health Foundation. Diabetes UK is seeking a program manager to deliver this.

Role: Year of Care Programme Manager

Salary: £45,000

Location: Diabetes UK (Camden, London)

The programme looks at how to make routine consultations between people with long term conditions and healthcare professionals much more collaborative, through care planning; and then at how to ensure that the services that have been identified to support this are available locally, through commissioning.  Focussing on diabetes care, the programme is working with three NHS sites to test this new approach to care and commissioning for people with long term conditions.

Based in Diabetes UK, the post holder will lead and manage the final year of the Year of Care programme.  Working individually and as part of a team, you’ll be responsible for supporting the pilot sites to maximise the potential of the programme and ensuring that the national strategic and operational objectives are met. You will be responsible for the day-to-day management of the programme and will work with the Chair of the Year of Care Board to ensure that regular progress reports are returned to the partner organisations, up to Director General level and Chief Executive level.  You will work closely with a wide range of internal and external stakeholders to ensure that the learning from the programme is captured and shared as widely as possible, and that the experiences and lessons from the pilot sites feed back into policy development and health reform.  The post holder will also coordinate a sub-project examining how commissioning for self management across a local community can most effectively support those involved in care planning.
